Prostate-specific antigen (PSA) is a glycoprotein that is produced by the prostate gland, the lining of the urethra, and the bulbourethral gland. Normally, very little PSA is secreted in the blood. 

PSA is present in small quantities in the serum of men with healthy prostates, but is often elevated in the presence of prostate cancer or other prostate disorders.PSA is not uniquely an indicator of prostate cancer, but may also detect prostatitis or benign prostatic hyperplasia.

Higher total PSA levels and lower percentages of free PSA are associated with higher risks of prostate cancer.Most prostate cancers are slow growing, so the utility of prostate cancer screening is marginal in most men with a life expectancy of less than 10 years.

Prostate cancer is the main cause of an elevated PSA level. But PSA levels increase with age and can reflect different prostate conditions. Other factors that may raise a person’s PSA level include: 

  • Prostate enlargement and inflammation (prostatitis).
  • Urinary tract infection.
  • Urinary catheter (tube) placement.

When total PSA levels are higher than the appropriate range, a person has an increased risk for prostate cancer. It does not show conclusively that a person has cancer. Hence comparison of the results of this test with results of a free PSA test can lead to proper conclusion.
